Madeira’s aim in this project is to bring healthcare professionals and citizens closer, equipping health institutions with innovative, interoperable, and intelligent solutions that enhance the delivery of care.  

By integrating the xShare Yellow button, patients will gain seamless access to their health information, while interoperability between healthcare providers is assured, fostering a single, secure, and standardized digital health market across Europe. 

This video discusses the implementation of the xShareYellow Button within the Madeira region’s digital health ecosystem, with the aim of enhancing healthcare interoperability and data sharing across Europe by aligning with international patient summary standards and the European Health Data Space regulations.

The Irish Adoption Site is a joint national partnership between the Department of Health (DoH) and the Health Service Executive (HSE). The DoH is responsible for the development of health strategy and the HSE is responsible for the implementation and operations of this health strategy.

The technology is designed to facilitate seamless sharing of patient summary information between citizens and healthcare professionals via the HSE’s mobile app. This button allows patients to easily access, view, download, and share their health data securely with trusted health professionals, thereby promoting patient empowerment and ownership of personal health information.

As an adoption site for the X-Share project, Fondazione Monasterio aims to leverage its expertise in managing complex clinical data and its advanced IT infrastructure to contribute to the development and implementation of the European Health Data Space (EHDS). Monasterio’s vision aligns with xShare’s goal of empowering individuals through better access and control over their health data, facilitating research, and improving healthcare quality.

This video discusses the pilot implementation of the xShare Yellow Button — a digital health tool integrated into FTGM patient portal in Italy. This initiative represents a significant step toward advancing digital health maturity by enabling patients to access and share their health information electronically, moving beyond traditional paper-based documentation.

Care plans provide an overview of the patient’s illness and key values that are significant for treatment and monitoring. The Care plan module can be used as a dialogue tool with the possibility of supporting the patient’s self-care and agreeing goals for the individual patient. Patients’ care plans can be shared with healthcare professionals at hospitals and municipalities via the national eHealth infrastructure, which contributes to coherent and coordinated patient care. 

The french adoption site objective is to demonstrate the interest of the xShare Yellow Button applied to “Mon espace santé” and patients pre-screening with CT-SCOUT. 

IDIKA, as the designated Hellenic NCPeH, will demonstrate the use of the xSHARE Yellow Button and with that will promote the European EHRxF in their national ecosystem of EHR vendors and digital health app developers. GΝΟΜΟΝ will be responsible for the implementation of the xSHARE Yellow Button PoC.
